NURSE-FAMILY PARTNERSHIP

HEALTHY FAMILIES = HEALTHY COMMUNITIES

UWGUC coordinates the Nurse-Family Partnership program in Union County. NFP is an evidence-based, community health home visitation program that helps transform the lives of vulnerable mothers pregnant with their first child.

Each mother in the program is partnered with a registered nurse early in her pregnancy and receives ongoing nurse home visits that continue through the child’s second birthday. Nurse-Family Partnership introduces vulnerable first-time parents to caring maternal and child health nurses.

This program allows nurses to deliver the support first-time moms need to have a healthy pregnancy, become knowledgeable and responsible parents, and provide their babies with the best possible start in life. The relationship between mother and nurse provides the foundation for strong families and lives are forever changed - for the better.
